The Nebraska Pet Hospice Quality of Life Assessment Tool is designed with privacy as a core principle. Your assessment remains private and is processed entirely on your device unless you explicitly choose to share anonymous data.
By default, all assessment data (pet name, your answers, and responses) is processed entirely in your web browser. Nothing is sent to our servers or stored in any database unless you explicitly opt-in to share anonymous data.
You can print or save your assessment summary directly from your browser. This creates a local copy on your device that you can share with your veterinarian.
You don't need to create an account, provide an email address, or share any identifying information to use this tool.
When website analytics is enabled, our hosting provider, Replit, processes page visits and usage events to help us understand how this tool is used. Events include starting and completing an assessment, moving between steps, saving or deleting an assessment, requesting a printout, opening assessment history, clicking the contact button, and successfully sharing anonymous responses. These custom events do not include pet names, scores, assessment answers, or other information you enter. Usage analytics is separate from the optional sharing of assessment responses described below.
On the summary page, you have the option to share your responses anonymously to help improve pet end-of-life care. If you choose to opt-in, we collect:
Even when you opt-in to share data anonymously, we NEVER collect:
While we do not collect your name, email, or other direct identifiers, please be mindful not to include identifying information in your text responses (such as your name, address, phone number, or other personal details). Your responses should focus on your pet's condition and your values, not on personal identifying information.
Anonymous data helps veterinary professionals better understand the challenges families face during end-of-life decision-making. This research can lead to improved support resources, better communication strategies, and more compassionate care for pets and their families.
Data sharing is completely optional. You can complete and print your assessment without ever sharing any data with us. The tool works exactly the same whether you choose to share or not.
Anonymous data may be used for:
All data transmitted to our servers (if you opt-in) is encrypted using industry-standard HTTPS protocols. Anonymous data is stored securely and is never linked to any identifying information.
We do not sell, rent, or share your data with third parties for marketing purposes. Anonymous aggregated data may be shared with veterinary researchers or educational institutions for the purposes described above.
